What Baby Cries Indicate?


As your baby becomes familiar with the new sounds, sights and stimulus; they will start responding to this world. But it is difficult for a new mother to identify the gestures of a baby. But, as you spend more time with your baby, you will understand clearly what your baby wants to communicate with you. Your baby may smile exactly in response to any of your gesture or action and it definitely indicates that your baby is in a happy mood.But,this is not the case with their cry. Give a close watch on your baby’s cry and you will understand that they cry in different ways for different needs. Here are some common indications that a baby may try to convey through their cries.

1.Time for a feed:
The only one option for a hungry baby to convey that it is time for a feed is crying. If they are dry and clean in a new nappy and still crying, chances are more that they are hungry. Your baby will stop crying as you feed them and make them comfortable.

2.Your baby is ill:
If you are not able to comfort and stop the cry of your baby with a feed or if they refuse feeding, you should check for the overall health status of your baby. Check for high temperature, swelling, rash, blood, breathing issues.

3.Time to change diaper:
One of the first things you have to do when your baby cries is to check their nappy. A wet and dirty diaper can irritate them and they can do nothing else than crying out of discomfort. If you are using a cloth diaper, always make sure that you change it in time.

4.Need a winding:
If your baby started crying after a feed, it may be due to gas production in their gut. All you have to do is winding your baby by keeping their belly either on your knee or on your shoulder. It is a good idea to keep a habit of winding your baby after each feed.

5.Ear infections:
If your baby cries excessively, check for other indications that they provide. If they cry while feeding or if they pull their ear while crying, suspect an ear infection. Your baby will find it painful to swallow and will continue crying as you try to make them comfortable with a feed.

6. Need nothing, but you:
Sometimes your baby cries when they want you to hold them. This will make them feel safe and secure. If they cry in a constant low pitch even after a good feed and a nappy change, reassure them with your presence and touch.

All About Lactose Intolerance In Toddlers


Milk is an important nutritional supplement for a toddler. As toddlers are at an age of getting weaning from breastfeeding, milk from other sources should be included in their diet. But, unfortunately there are chances for some toddlers to develop gastrointestinal disturbances after consuming milk. The reason for this is mainly lactose intolerance. According to Dr.Richard Grand, director of the Center for Inflammatory Bowel Disease, lactose intolerance generally occur after two years of age. Lactose intolerance can occur in certain babies from birth itself, which is genetic.

It is important to distinguish lactose intolerance from allergies to animal milk protein. Once the condition is ruled out to be lactose intolerance, you have to take some extra care on your toddler’s diet management. Here, we may discuss the causes, symptoms, diagnosis and management of lactose intolerance.


All About Lactose Intolerance In Toddlers  


1.Causes:
Lactose intolerance is a condition where the body does not produce enough enzyme called lactase, which is needed to break down lactose, a sugar in milk. Lactase is needed to convert lactose to glucose and galactose, which are absorbed easily. When there's not enough lactase in the body, lactose passes into the large intestine where bacteria ferment it into gases and acids. This results in gastrointestinal disturbances.

2.Symptoms:
Symptoms may vary from mild, moderate or severe depending on the tolerance of the toddler to lactose. General symptoms include cramps, abdominal pain, gas, and diarrhoea in about 30 minutes to 2 hours after taking any foods or drinks that have lactose. Lactose intolerance will not cause vomiting, but a food allergy can.

3.Diagnosis:
Strict dietary avoidance of lactose and continuous follow-up of the toddler for absence of suspected symptoms is the main step in clinical diagnosis of lactose intolerance. Certain lab tests like lactose breath hydrogen test will help in diagnosing lactose intolerance and in differentiating it from animal milk protein allergy.

4.Management:
Avoiding dairy products is not the remedy for all toddlers. Regulate the diet of your toddler under the supervision of a registered dietitian or a physician. Try short-term lactose restriction and introduce lactose slowly in an increasing amount. Including regulated amount of dairy products in your toddler’s diet is recommended as it may not result in severe symptoms. Replace dairy products with other sources of calcium and vitamin D if your toddler shows extreme discomfort.

5.Outlook:
It is difficult to determine how much lactose your toddler can tolerate. So, be patient and monitor your toddler’s dietary intake of milk and other dairy products. This will gradually let you know the tolerance limit of lactose for your toddler.

Remember that lactose intolerance is not an allergy to milk. Regulating the amount of lactose intake is important in the management of lactose intolerance.

The cost of production of most of the drugs is many times less than the retail price (MRP) printed on it. Generic medicines are not new types of medicines, these are abundantly available everywhere in India. These are drugs which have come out of the patent duration of 20 years and now, apart from the original innovator company, any other pharmaceutical manufacturer can produce this salt after getting due permission from the drug control authorities. In fact, 97% of the drugs in the Indian market are generics (or branded generics). But even these are sold at high prices, because they are prescribed by doctors and supplied by manufacturers under brand names. Branding limits the choice for the patient, kills competition and creates artificial monopoly, and enables the manufacturers to charge a very high MRP.

The doctor plays a key role not only in prescribing drugs by generic name but also in convincing the patients that these drugs are 100% identical to commonly used brands in quality, ie, they have the same identity, purity, strength, bioequivalence and effectiveness, provided the doctor himself is convinced of these facts. Even when you open a low-cost generic drug shop the patient won’t get the price benefit unless the doctor is inclined to help him.
The patient must also be made to understand that although he is getting a particular drug at a very low price compared to the printed MRP or his doctor’s favourite brand, it is in no way different or inferior. It will have the same effect.


Steps To Open& Operate A Fair Price Generic Medicine Shop

The hospital management or the local self-government or NGO or any other private entity should discuss the issue, evolve a consensus, take doctors and staff into confidence and adopt a resolution to provide low-cost drugs to the citizens. Take from the doctors the list of drugs which are commonly prescribed by their generic/salt name (start with 100-200 drugs which are not available in hospital supply). Let doctors also decide the name of 15-20 companies which they consider as “good”.

Invite open tender by generic name or take drug lists of reputed drug companies (which doctors have indicated) from the authorised local stockists (the wholesalers) of these companies. Apart from the offer price the stockist lists usually also shows MRP which may be 5 to 10 times the offer price. To avoid misbranded or spurious drugs always get these from a stockist authorised by the company. Also insist for the authorisation certificate, a valid bill/cash memo and certificate of analysis (test report) for every batch of each drug. This will ensure quality standards.

Compare the prices of different companies and prepare a comparative statement. Choose the L1 (best buy) and purchase these generic drugs. A fair price generic drug store can be opened in any suitable vacant room near/in the hospital. A registered pharmacist is required to run the store.
 The licence for the shop is to be obtained from the office of the local drug control authority, for a licence fee of about Rs 3,000 (one time for 5 years).

The generic drugs received from the authorised suppliers can be sold at 10-20% profit margin which is charged at the purchase price, which will help to meet day-to-day expenses and the remuneration of the pharmacist, and no external financial help is required.
A computerised list of the available generic drugs may be displayed prominently outside the shop. Preferably the list should show the rate at which the drug is actually sold (CP+10%). You may also compare the selling price with the MRP printed on the drug, which will educate the patients about drug prices. 

Similarly, surgical items like IV sets, blood transfusion sets, needles, canulas, suture items etc can be sold. Orthopaedic implants, eye lenses, cardiac stents etc can be sold at about one-fourth of the prevalent market prices. The mere availability of low-cost drugs will not help, until the doctors can be persuaded to prescribe either the generic (salt) name of the drugs or at least prescribe those brands which are of low cost and are available at the store.

Then the patient must be properly guided and counselled to purchase drugs from the low-cost generic drug store. In Rajasthan about 72 fair price generic medicine shops are operational in medical college hospitals, district, sub-district and satellite hospitals by the name of “Life Line Drug Stores” and are operated by Rajasthan Medicare Relief Societies, which are in fact the “Rogi Kalyan Samiti” that is the local hospital management committees.


If the state government or local body or charitable trust is ready to bear the cost (which is very low, about Rs 10 per OPD patient), then these generic drugs can be given to poor/all patients free of cost as is being done by the Government of Rajasthan, through 15,000 free drug distribution Centres, right from medical college hospital to PHCs and sub-centres. How To Stop SMS Fights


A face-to-face quarrel is far better than an SMS fight. Avoiding SMS fights are important because predominantly harsh words and languages are used when one need not have to see the other's reaction. At the same time, another important point to be kept in mind is that you are giving a clear chance for the other person to keep a record of your thoughts and words. As SMS goes as a record, it can be used at any later time as evidence of your character and it can always do harm to your relationship. Curry leaves with  botanical name Murraya koenigii, are an inevitable part of Indian cooking where almost all dishes starts or ends with it, either for seasoning or for garnishing. Other than cooking curry leaves are used as ingredients in many Ayurvedic preparations. It is a good idea to grow curry leaves in your kitchen garden so that you can enjoy cooking with pesticide free fresh leaves. Curry leaf plants are fast growing deciduous shrub which grows well in temperature around 65 °C or slightly above. It can be grown successfully either from the seeds or from the stem, if proper care is given. Here are some points to keep in mind if you are planning to plant a curry leaf plant in your garden.

1.Plant outdoors: Since curry leaves need sunlight, it is better to grow them outdoors. If you are living in apartments, prefer any area where you can provide direct sunlight for the plant. At the same time, it is important to protect it from very high temperatures during hot summer months.

2.Grow from seeds: Curry leaves can be easily grown from its seeds. Ripe seeds need about 20°C to germinate. While using seeds, you should remember that it may take a little longer time for the curry leaf plant to grow.

3.Plant the stem: Stem cuttings are the easiest way to plant a curry leaf plant. Take some of the twigs in the semi-ripe stage which are not very hard and woody. After cutting the stem at the node, insert it to a few centimetres down into the ground or into the pot with some leaves above the surface.

4.Trim it regularly: It is very important to trim your curry leaf plant in order to get a fast and thick growth. Trimming will provide an ample supply of fresh and young leaves. If you leave the plant without proper trimming, it will outgrow any space easily.

5.Pest control: Usually pests are not a great concern when considering curry leaf plant, since the leaves are  constantly being removed. It is better to go natural instead of using chemical pesticides. It is a good idea to sprinkle salt water on the leaves once in two weeks.

6. Watering cycles: Curry leaf plant prefer a well-drained soil to flourish well. So, it is recommended to allow the soil to dry a bit in between each watering session. Watering the curry leaf plant is important during summer when the soil will get dried up easily.


7.Using fertilizers: Bio-fertilizers are always preferred for the curry leaf plant. Regular application of fertilizers is more important for plants that are cultivated in pots.Remember not to give high dose of fertilizers to young plants.

2.Hydrogenated or partially hydrogenated cooking oils:  Avoid using oils labelled as hydrogenated or partially hydrogenated because they contain high amounts of trans-fat. Trans-fat is always considered responsible for cardiac problems due to blockage of arteries.

3.Canola oil: Canola oil is also included in the list of unhealthy cooking oils which needs a second thought before using for cooking. Canola oil is harmful because it is highly refined. Canola oil undergoes many processes which involve high temperatures or chemicals like caustic refining, bleaching and degumming.

4.Soy oil: Soy oil should never be your first choice if you want to stick to a healthy cooking oil. The increased amount of omega 6 fat in soy will result in internal inflammations. Vegetable oils like soy oils should be avoided in your cooking since it is unhealthy.

5.Safflower oil: Safflower oil contains inflammatory type fats. Safflower oil should be avoided for deep frying as it is unhealthy. Increased consumption of safflower oil may result in unbalanced levels of Omega-3 and Omega-6 fats in our body. Safflower oil is not considered eligible to have a place in a  healthy cooking style.

Avoid using unhealthy cooking oils to stay healthy. At the same time, limit the amount of cooking oil you use in your food to a minimum, even if it is one among the healthiest oils.
